Medical Social Worker

St. Francis Hospital is seeking a compassionate and experienced Medical Social Worker to join our interdisciplinary care team. This role plays a vital part in supporting patients and families throughout hospitalization by facilitating discharge planning, addressing psychosocial needs, and coordinating care transitions. The MSW ensures that care is respectful of each patient's values, preferences, and beliefs, while promoting safe and timely discharges.

What You Will Do:

  • Monitor patient progress and document findings related to social services and discharge planning.
  • Conduct timely interviews with patients and families to assess psychosocial, cultural, and emotional needs.
  • Collaborate with multidisciplinary teams to develop and implement discharge plans.
  • Coordinate care in home or alternate settings, including SNFs, rehab centers, and psychiatric facilities.
  • Assess and intervene in cases of suspected abuse, neglect, or domestic violence.
  • Assist patients and families with Medicaid applications and long-term care planning.
  • Facilitate meetings between patients, families, and providers to address psychosocial and economic concerns.
  • Identify patients at risk for rehospitalization and mediate barriers to successful discharge.
  • Maintain accurate documentation in medical records and clinical databases (e.g., MIDAS).
  • Ensure compliance with hospital policies and external regulatory standards (e.g., JCAHO, Department of Public Health).

Minimum Qualifications:

  • Education: Master's Degree in Social Work (MSW) required.
  • Experience: Minimum of 1 year providing social work services in an acute care setting preferred.
  • Skills: Basic computer literacy and proficiency in Microsoft Office. Experience with clinical database software; MIDAS knowledge a plus. Strong leadership, interpersonal, oral, and written communication skills.
