St. Francis Hospital is seeking a compassionate and experienced Medical Social Worker to join our interdisciplinary care team. This role plays a vital part in supporting patients and families throughout hospitalization by facilitating discharge planning, addressing psychosocial needs, and coordinating care transitions. The MSW ensures that care is respectful of each patients values, preferences, and beliefs, while promoting safe and timely discharges. What You Will Do. Monitor patient progress and document findings related to social services and discharge planning. Conduct timely interviews with patients and families to assess psychosocial, cultural, and emotional needs. Collaborate with multidisciplinary teams to develop and implement discharge plans. Coordinate care in home or alternate settings, including SNFs, rehab centers, and psychiatric facilities. Assess and intervene in cases of suspected abuse, neglect, or domestic violence.
