Job Title: Care Navigator (Hybrid-Medical Assistant- with transition to in person), Psychiatry and Mental Health Team Reports to: Care Navigator Manager- Mental Health Team Employment Type: Full-Time. Hybrid (Remote & In-Person at Old Fourth Ward, Atlanta, GA), Must reside in Georgia. Monday, Tuesday, Thursday, 7:30am-6pm. Position Summary Step into a pivotal role as a Care Navigator on our Psychiatry and Mental Health team, where your contributions will directly support both providers and patients with compassion and efficiency. As a Care Navigator, you'll serve as a key patient support liaison, often becoming the first point of contact for new members. Company Benefits We offer a competitive benefits package to our employees (available to team members working 30 hours per week): · Medical, Dental, and Vision · 401(k) with Match · Generous PTO, Paid Parental Leave, Company Holidays · Flexible Schedule and Work/Life Balance Key Responsibilities · Conduct virtual and in-person patient intake interviews at our Old Fourth Ward location to gather medical history and current mental health concerns. · Schedule and coordinate telehealth and in-person appointments for patients with mental health professionals in the Old Fourth Ward area. · Assist in managing and updating electronic health records (EHR) with accurate patient information. · Provide administrative support by handling phone calls, emails, and other communication related to patient care. · Facilitate communication between patients and mental health providers, ensuring timely follow-ups and responses. · Prepare and distribute educational materials and resources to patients regarding mental health conditions and treatments. · Monitor and manage patient portals, ensuring patients have access to necessary information and services. · Assist with billing and insurance claims processing for mental health services. · Conduct follow-up calls or messages to patients to ensure treatment adherence and gather feedback. · Maintain confidentiality and comply with HIPAA regulations and other relevant legal requirements. · Support mental health professionals in preparing documentation and reports for patient care planning. · Assist in triaging patient inquiries and managing crisis situations by directing them to appropriate resources or personnel. Requirements: Qualifications · Certified Medical Assistant (CMA) or equivalent qualification required. · Minimum of 2 years' experience working in a healthcare setting required. · Minimum of 1 year of experience in psychiatry or behavioral health strongly preferred. · Must be able to reliably commute to Old Fourth Ward, Atlanta, GA for in-person work. · Demonstrated ability to maintain patient confidentiality and adhere to HIPAA regulations. · Comfortable using telehealth platforms and digital communication tools. · High-speed internet connection and a quiet, dedicated workspace for any remote work . · Familiarity with scheduling software and patient management systems. · Fluent in English; bilingual skills are a plus.
