Program Overview:

Our Oak Street Health@HOME program offers exceptional primary care to Medicare patients in their homes through a collaborative, team-based approach. Our Care Teams consist of a Provider, Medical Assistant/Scribe, Behavioral Health Specialist, and Mobile Operations Coordinator.

Role Description:

The Medical Assistant/Scribe plays a vital role in the in-home care team, enhancing logistics, clinical care, and documentation for our home-based care program. Daily responsibilities include:

• Preparing daily visit schedules and planning logistics
• Accompanying Providers on home visits
• Checking vital signs, administering vaccinations, and conducting point-of-care tests
• Documenting patient encounters accurately and comprehensively in the EMR, including HPI, assessment, plan, and physical exam
• Improving clinical documentation and supporting follow-up care
• Participating in daily huddles and interdisciplinary team meetings
• Managing orders and referrals effectively

What We're Looking For:

We seek individuals with a genuine passion for primary care and a commitment to serving vulnerable patients. Essential qualifications include:

• Certification as a Medical Assistant
• CPR or BLS Certification
• Minimum of 1 year of relevant experience
• Flexibility to travel within the service area
• Proficiency with Electronic Medical Records and basic computer functions
