Role Description

This is a full-time on-site role for an MDS Coordinator located in the Dunedin, FL 34698. The MDS Coordinator will be responsible for conducting and coordinating the Minimum Data Set (MDS) assessments, developing and implementing care plans, and ensuring compliance with regulatory requirements. The role involves working closely with residents, their families, and healthcare professionals to create and maintain accurate and comprehensive documentation. The MDS Coordinator will also monitor and evaluate the effectiveness of care plans, making necessary adjustments to optimize residents' health and well-being.

Qualifications

Registered Nurse RN required

• Experience in MDS and geriatric nursing
• Skills in creating and implementing care plans
• Strong organizational and time management skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively with a multidisciplinary team
• Knowledge of state and federal regulations related to long-term care
• Proficiency in healthcare software and electronic health records (EHR)
