Responsibilities:

As a Maternity Care Coordinator, you will play a crucial role in supporting maternity members in achieving their optimal health through effective care coordination and case management. Your responsibilities will include:

  • Conducting thorough assessments to determine care needs for assigned members.
  • Completing comprehensive, person-centered assessments covering physical, psycho-social, and environmental health.
  • Identifying barriers to care and developing appropriate interventions.
  • Coordinating a range of services including physical, behavioral health, and social supports.
  • Managing medications through regular reconciliation and supporting adherence.
  • Creating personalized care plans to address member needs and resolve barriers to health.
  • Setting specific short- and long-term goals with members and their caregivers.
  • Implementing targeted interventions based on individual member needs.
  • Documenting progress and scheduling follow-ups as needed.
  • Ensuring seamless care coordination among providers and services.

Experience:

  • Minimum of 3 years of experience in maternal health settings (acute care, community health, or ambulatory care).
  • Preferred case management experience within a managed care organization.
  • Proven ability to work independently, adapt to change, and collaborate effectively in a fast-paced environment.
  • Cultural competence and skill in engaging diverse populations.
  • Strong organizational and time management abilities to prioritize effectively.
  • Knowledge in assessing members' situations, developing care plans, and promoting self-management.

Education/Licensure:

  • Current, unrestricted Ohio RN license in good standing is required. Associate degree in nursing required; Bachelor's degree preferred.
  • Valid driver's license and car insurance are necessary.
