The MAT Clinical Care Coordinator is responsible for the coordination and care of patients receiving Medication Assisted Therapy (MAT) for opioid addiction. Teaming with a Registered Nurse, the position provides health home services to a panel of over 100 MAT patients in coordination with prescribing buprenorphine providers and Behavioral Health clinicians at CHCB. This position will also coordinate with other treatment providers from whom patients are receiving services. As part of embedded support service, the MAT BH Clinical Care Coordinator will provide for the planning, coordination and transition of care. The Care Manager will also provide case management services for other psychosocial issues. Basic Qualifications
- A Masters Degree in a Mental Health field
- 3 - 5 years experience in a mental health setting or substance treatment program
- Prior experience in a physician practice highly preferred.
- BLS certified
- Ability to utilize cognitive-behavioral and dialectical interventions.
- Understanding of mind body and biopsychosocial implications of health.
- Strong written/verbal, organizational, and problem solving skills
- Experience with addictions assessment, treatment placement criteria, and treatment modalities.
- Ability to present in a professional manner benefiting the organization.
- Knowledge of the principles, standards, and ethics of the mental health field
- Knowledge of transference and counter transference.
- Willingness to work flexible hours, including evenings.
- Ability to maintain credentials and privileges as a Licensed Alcohol and Drug Counselor (LADC).
