Manager of Care Delivery – RNWe are seeking an experienced and motivated Manager of Care Delivery – RN to join a mission-driven healthcare organization in Ridgewood, NY! This leadership opportunity offers competitive pay, strong benefits, and the chance to make a lasting impact while leading a team of skilled community health nurses dedicated to high-quality, patient-centered care.ScheduleMonday – Friday | 8:30 AM – 5:00 PM | Fully onsiteCompensation$120,000 – $125,000 annually (DOE)Daily Responsibilities For Manager Of Care Delivery

• Lead and mentor a dedicated team of 4–5 community health nurses, fostering excellence and teamwork.
• Coordinate care delivery, manage staffing and resources, review documentation, and ensure regulatory compliance.
• Collaborate with interdisciplinary teams to support long-term wellness and independence for patients.

Required Qualifications For Manager Of Care Delivery

• 5 years of clinical experience
• 3 years of home care experience
• Active NY Registered Nursing license
• Leadership experience
