About the Opportunity:
A well-established, full-service acute care hospital in the heart of the Inland Empire is seeking an experienced Manager of Case Management to join their dynamic team. This leadership role plays a critical part in ensuring effective and timely care coordination, proactive discharge planning, and regulatory compliance across all inpatient units.

Key Responsibilities:
- Lead the day-to-day operations of the Case Management department, including staffing, scheduling, performance oversight, and staff development.
- Support patient throughput by evaluating direct admits and assisting with timely discharge planning.
- Build and maintain positive relationships with physicians, health plans, payers, patients, and families.
- Identify and escalate delays in care progression; partner with facility leadership to resolve barriers.
- Promote safe, appropriate discharge plans that align with hospital goals and patient/family needs.
- Serve as a resource and support to Case Management staff through rounding and direct engagement.
- Monitor regulatory compliance and ensure adherence to federal and non-federal program guidelines.
- Participate in a shared leadership on-call rotation to address emergent after-hours issues.
What You Bring:
- BSN required; MSN preferred
- Current California RN License (pending license considered)
- 3-5 years of Case Management or Nursing Leadership experience in an acute care hospital
- Strong working knowledge of InterQual and Milliman Care Guidelines
- Deep understanding of hospital reimbursement systems (Medicare, Medi-Cal, Managed Care, private payers)
- Proactive approach to discharge planning and patient/family communication
- Strong leadership, collaboration, and critical thinking skills
Department Overview:
- Leadership reports to the Director of Case Management
- Responsible for managing 15-18 FTEs including RN Case Managers and Case Management Assistants
- Supported by one Administrative Assistant
- All inpatient units are served by Case Management
- Total hospital capacity: 542 beds
