Job Description:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a full-time, permanent position at our 5-star rated, 30-bed Skilled Nursing Facility (SNF). As an LPN, you will play a key role in providing high-quality care to our residents while working the 5:00 PM to 5:30 AM shift, three days per week, with every other weekend and some holiday shifts required.

Key Responsibilities:

• Provide high-quality nursing care to residents in accordance with care plans and facility policies.
• Administer medications, treatments, and provide general nursing care to residents.
• Monitor resident conditions, document observations, and report changes to the healthcare team.
• Collaborate with doctors, RNs, CNAs, and other healthcare professionals to ensure optimal care.
• Maintain accurate medical records using the PCC EMR system.
• Ensure a safe and comfortable environment for all residents, following all infection control guidelines.
• Assist in resident assessments, care planning, and evaluations of progress.

Qualifications:

• LPN license required (current and valid in the state).
• Minimum of 1 year of LTC experience is preferred.
• Experience with PCC EMR system is highly preferred.
• Strong communication skills and ability to work as part of a collaborative team.
• Compassionate, patient, and dedicated to providing excellent care to residents.
• Ability to work 3 shifts per week with every other weekend and some holidays.

Compensation & Benefits:

• Hourly pay: $25 - $30 per hour, based on experience.
