Summary:

The Lung Nodule Navigation Nurse is a professional Registered Nurse with specific clinical knowledge, who offers individualized assistance to patients, families and caregivers. The Lung Nodule Navigation Nurse acts as a care coordinator ensuring patients receive timely and appropriate follow-up care, including screenings, diagnosis, and treatment. The Navigation Nurse works to coordinate high quality, person-centered care through effective communication and collaboration with the interdisciplinary team throughout all phases of the patient experience.

The Lung Nodule Navigation Nurse is a critical member of the care team identifying and eliminating barriers to receiving appropriate care and providing education and resources to facilitate informed decision-making. The Navigation Nurse acts as a liaison between patients, caregivers and the care team to optimize patient outcomes and facilitate efficient care transitions while serving as a resource to physicians and other healthcare professionals.

Responsibilities:

EDUCATION

  • Current New York State RN license required

  • Bachelor's degree in nursing from an accredited school of nursing required

  • BLS certification required

EXPERIENCE

  • Four (4) years nursing experience, with a minimum of 2 years in acute care required

  • Previous experience in case management, patient navigation or care coordination preferred

KNOWLEDGE, SKILL AND ABILITY

  • Excellent communication and listening skills, communication skills both orally and written for a variety of audiences

  • Proficient technology skills to utilize the EMR, computer programs and technology to develop presentations, trend data and identify opportunities for improvement

  • Highly organized, able to implement, coordinate and direct a variety of programs and services

  • Ability to take initiative and provide self-directed leadership to ancillary team while driving results

  • Able to work collaboratively with other healthcare professionals and providers to deliver patient-centered, interdisciplinary care
