Summary:
The Lung Nodule Navigation Nurse is a professional Registered Nurse with specific clinical knowledge, who offers individualized assistance to patients, families and caregivers. The Lung Nodule Navigation Nurse acts as a care coordinator ensuring patients receive timely and appropriate follow-up care, including screenings, diagnosis, and treatment. The Navigation Nurse works to coordinate high quality, person-centered care through effective communication and collaboration with the interdisciplinary team throughout all phases of the patient experience.
The Lung Nodule Navigation Nurse is a critical member of the care team identifying and eliminating barriers to receiving appropriate care and providing education and resources to facilitate informed decision-making. The Navigation Nurse acts as a liaison between patients, caregivers and the care team to optimize patient outcomes and facilitate efficient care transitions while serving as a resource to physicians and other healthcare professionals.
Responsibilities:
EDUCATION
Current New York State RN license required
Bachelor's degree in nursing from an accredited school of nursing required
BLS certification required
EXPERIENCE
Four (4) years nursing experience, with a minimum of 2 years in acute care required
Previous experience in case management, patient navigation or care coordination preferred
KNOWLEDGE, SKILL AND ABILITY
Excellent communication and listening skills, communication skills both orally and written for a variety of audiences
Proficient technology skills to utilize the EMR, computer programs and technology to develop presentations, trend data and identify opportunities for improvement
Highly organized, able to implement, coordinate and direct a variety of programs and services
Ability to take initiative and provide self-directed leadership to ancillary team while driving results
Able to work collaboratively with other healthcare professionals and providers to deliver patient-centered, interdisciplinary care
