We CARE for our patients like they are our own FAMILY.$5000 SIGN ON BONUSLPN Clinical Field Staff Liaison - HomecareSchedule: Monday - Friday 8:00am - 5:00pm with no weekends or on-call requirementsAgency: The OSU Wexner Medical Center Homecare Must Have Home Health ExperienceTHIS IS A HYBRID ROLE WORKING 3 DAYS IN OFFICE AND 2 DAYS WORK FROM HOME AFTER 90 DAYS OF TRAININGSummaryThe LPN Clinical Field Staff Liaison (CFSL) provides support and acts as a resource to clinicians in the field. The CFSL works in the field with staff and patients and completes visits as needed. ASHN adopts the LPN scope of practice as outlined by the state for its licensed practical nurse personnel. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.Qualifications & AttributesLicensed Practical Nurse with current license in the state of employment. Compact licensure, for states that participate, is required to be obtained within first 30 days of employment for additional support in all ASHN affiliated agencies.Two years of experience as an LPN in an acute care setting. Minimum one-year home health experience.Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.Disciplined style of work ethic. Ability to prioritize and be timely. Works efficiently.Ability to follow directions and work as a team member.Major Areas Of ResponsibilityServe as a resource for field staff pertaining to clinical issues and/or concernsFacilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to managerWork in the field with staff and patients as neededReview Field staff STEPS performanceMaintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and familiesAssist with monthly Case Manager, Nurse and Therapy meetingsFocus on Outcome Monitors as assignedCompletes field visits as neededParticipates in managing cost of good working with monitoring supply costs and visits per episodeKnowledgeable of Clinical Review workflowReview relevant reportsReviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesMaintains active LPN license in state of employmentParticipates in on-call, evening/weekend shifts and provides patient care, per agency needsParticipates in in-service program developmentMaintains a professional appearance as a representative of the companyUnderstands and practices policies and procedures for agency operationEssential FunctionsArrives at assigned location on scheduled work day. Works according to designated hoursCurrent applicable license as a License Practical Nurse.
