The overall duties and responsibilities of this role may vary slightly based on specialization and from setting-to-setting, depending on the market and size, location and patient volume of his/her assigned center and/or Specialist. They will guide patients through complex and routine medical and social challenges in a manner that fosters trust and respect between the patient and the care team. The LPN Specialist is an integral member of a Physician-led care team that is focused on providing excellent and comprehensive primary care for a specific population of patients.LPN Cardiology Specialist will play a crucial role in supporting our mission to urgently pursue more good days for our patients by preventing hospital visits through proactive cardiac care management.Essential Job Duties/ResponsibilitiesPatient Care & Monitoring:
- Assist with cardiac-focused patient visits and assessments
- Monitor vital signs, especially blood pressure and heart rate patterns
- Support EKG administration and basic interpretation under RN/provider supervision
- Help with medication reconciliation for cardiac medications
Core Cardiac-Specific SkillsEKG Competency:
- 12-lead EKG placement and acquisition
- Basic rhythm recognition (normal sinus, atrial fibrillation, bradycardia, tachycardia)
- Understanding of common cardiac abnormalities in our senior population
Vital Signs & Assessment
- Accurate blood pressure measurement techniques (manual and automated)
- Recognition of orthostatic changes
- Pulse assessment including irregularities
- Oxygen saturation monitoring
- Weight monitoring for fluid management
Point-of-Care Testing
- INR testing and documentation for anticoagulated patients
- Basic metabolic panels
- BNP/NT-proBNP testing when available
- Glucose monitoring
Patient Relationship Building
- Help build trust between patients and the care team
- Provide respectful, dignified care to our senior population
- Support patients with limited resources in understanding their cardiac care plan
- Use 6th-grade level communication when educating patients
Knowledge, Skills And Abilities
- Exceptional knowledge and understanding of general Practical Nursing protocols, functions, practices, processes, policies & procedures
- Knowledge and understanding of medical products, terminology, services, standards, policies and procedures
- Exceptional knowledge and understanding of safety and sanitation protocols. Must be aware of personal and patient safety at all times
- Ability to effectively collaborate with team members, including physicians, clinicians, market leaders, center managers and front desk team staff
- Ability to manage complex patients requests from providers
- Ability to make some independent workload decisions
- Ability to maintain effective and organized systems to ensure timely patient flow
- Ability to exercise patience, compassion and empathy for patients and family members
- Ability to act calmly in busy or stressful situations
- Skill in operating phones, personal computer, software and other IT systems. Must be detail-oriented to ensure accuracy of reports and data
- Excellent clinical skills to identify, diagnose and appropriately resolve patient issues
- Excellent oral and written communication skills
- Good non-clinical skills to assist with dressing, physical exercise, restroom visits, etc. as needed
- Good time management to ensure tasks are completed timely and efficiently
- Good motor skills to effectively calibrate equipment, manipulate intravenous equipment, administer injections, position and transport patients when needed
- Mindset focused on solving problems for patients and achieving team goals
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software
- Ability and willingness to travel locally and regionally up to 50% of the time
- Spoken and written fluency in English
Education And Experience Criteria
- High school diploma or equivalent education (GED) required
- Active LPN license in the state of practice
- 1 year of LPN experience REQUIRED
- 1 year of Cardiology experience preferred
- 3 years of clinical experience preferred
- Medication reconciliation certification preferred
- Cardiac rehabilitation support training Preferred
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
