This position is field-based with in-person visits to member homes or facilities required up to 75% of the time, with 25% remote/home-based work. Candidates should reside in Pinellas County, FL to be considered for the role.

Position Purpose: As a Long-Term Services and Supports Care Coordinator, you will assist in developing, assessing, and coordinating comprehensive care management activities to ensure quality and cost-effective healthcare outcomes. You will play a crucial role in creating personalized service care plans and educating members and their families about available services and benefits.

  • Evaluate member needs and recommend or facilitate plans for the best healthcare outcomes.
  • Assist in developing ongoing long-term care plans and identify necessary providers, specialists, and community resources.
  • Coordinate effectively between members, families, caregivers, and healthcare providers to ensure accessible services.
  • Provide resource support for members and their families on various needs, including employment, housing, and independent living.
  • Monitor care plans and member status, providing recommendations based on changing needs.
  • Engage with long-term care providers to ensure member needs are met.
  • Collect and document member information and care management activities for compliance.
  • Perform home visits to assess member needs and collaborate with healthcare providers.
  • Educate long-term care members and families on services, benefits, and healthcare procedures.
  • Provide feedback to leadership on opportunities to enhance quality of care and service delivery.
  • Perform other duties as assigned and comply with all policies and standards.

Education/Experience: Requires a Bachelor's degree and 2 - 4 years of related experience, or equivalent experience demonstrating applicable knowledge and skills.

Pay Range: $26.50 - $47.59 per hour
