- Start Date: They can start in 2 weeks from being submitted to Humana and with ALL CAQH items in their profile updated
- Realistic timeframe for Creds: 30-60 days to credential with CareTalk and Humana
- Contract length: Through the end of the year (working on having them ongoing)
- Schedule: At LEAST 16hrs per week, M-F, Saturdays, an average of 16hrs is fine too
- Shift Times: M-F 8am-8pm and Saturdays 9-8, please send their availability and they will work them into the timeslots needed

- Requirements: Must have telehealth experience (Medicare patient experience preferred), Please see in files CTH technical requirements, they MUST have their own Windows or Apple computer, CareTalk will NOT provide them with one if they do not have, Must be in the US when conducting visits, MUST BE PECOS certified, Must have an 855i in the state where they reside, DEA NOT REQUIRED, CLEAN BACKGROUND REQUIRED, NO MALPRACTICE
Additional Details:
- Procedures: Subset of patients that have not had an Annual Wellness Visit in the last 12 calendar months to help bridge while identifying and closing potential gaps in care. This will evolve over time & could include at-home testing kits (A1C, Colo-kit) and providers writing orders for care items like mammograms & bone density scans to further expand gap closure for these patients. All results delivered back to the ordering physician, all normal results will then be relayed to the patient, any ABNORMAL results, the Provider will have a visit, discuss results and they will refer them out for treatment. We are NOT treating, the sole purpose of these appointments is to identify gaps since their last annual wellness visit, and write orders to fill those gaps, and work with Humana to get these patients caught up on their wellness visits.
- Provider Responsibilities: Conduct an Annual Wellness visit and if gaps in care are presented, follow proper orders depending on which gap is being identified. Visits range from 10-20 minutes in length. There will be no labs involved with this preventative care visit. Health risk assessments will be completed prior to the visit. Medications and vitals will be prepopulated in the platform
- Providers are expected to: Review HRA, Complete AWV and address applicable quality gaps, PPPS - Recommend additional screenings or tests as needed (Future State)
- Visit Types: 100% virtual Telehealth Synchronous (live video and/or phone calls) Patients will be at home.
- Patient Census: Dr. will give their availability and we will fill the time slots based on that. 3-6 visits per hour
- Patient Mix: Older people in Medicare/ Medicaid, appts may last 20 min or longer
- Support Staff: NA
- EMR: All patient interactions and notes must be documented in CT360, our telehealth platform on the same day. Phone consultations via Call shaper, and move to video call if applicable
