• Monday, Wednesday, Friday schedule -- 9am - 4:30pm
• 8 - 13 patients per day
• Patient age range: 5 - 99 years
• Experience with physical/annual wellness assessments
• Focus on chronic conditions management
• Hospital privileges available
• 2 years of NP/PA experience required
• BLS certification required
