Locum Physician Assistant (PA) - Family Practice in Ridgewood, NY - AAS Locum Tenens : Job Details

We are seeking a Locums Nurse Practitioner or Physician Assistant to join our geriatric care team in Ridgewood, NY. This is a flexible, part-time or full-time role providing in-home primary care to PACE (Program of All-Inclusive Care for the Elderly) participants. The ideal candidate is experienced in home-based or geriatric care, patient-focused, and comfortable working independently in the field. Position Highlights: Flexible Schedule: 2–4 days per week, Monday–Friday No nights or weekends Low patient volume: 3–5 patients per day Hourly Compensation: $75–$80 per hour Opportunity to make a meaningful impact on the health and well-being of medically complex older adults Competitive Compensation: $75–$90 per hour (not per patient) – paid for full day regardless of patient volume. Responsibilities: Provide comprehensive primary care services in participants' homes Conduct routine and urgent visits, including history, physical exams, medication reconciliation, and care planning Collaborate with interdisciplinary PACE care team to ensure coordinated, high-quality care Document clinical encounters in a timely and accurate manner Support chronic disease management, preventative care, and advance care planning Requirements: Active Nurse Practitioner or Physician Assistant Board Certification Medicare & Medicaid Provider ID – Required Minimum 2 years of clinical experience preferred, ideally in geriatrics, primary care, or home care settings Spanish-speaking candidates strongly preferred, but not required Strong interpersonal and communication skills Comfortable practicing independently in home-based environments This is a rewarding opportunity for a compassionate and experienced NP or PA seeking a flexible, field-based role with a low daily volume and no after-hours commitment.
