Job Description:Seeking a qualified and compassionate Social Worker to provide psychosocial assessments, develop care plans, and connect individuals and families with community resources. This full-time position supports patient care coordination and promotes independence through strengths-based, family-centered services.

Key Responsibilities:

• Perform psychosocial evaluations and maintain care plans
• Provide family-centered support to meet patient needs
• Coordinate care with interdisciplinary teams and external agencies
• Maintain accurate and timely documentation
• Participate in quality improvement and staff development activities

Qualifications:

• Bachelor's degree in Social Work required; Master's preferred
• Minimum 1 year of experience in public health or a related field
• Strong understanding of local resources and culturally competent care
• CPR and Mandatory Reporter certifications (or obtain within 30 days)
• Valid driver's license and reliable transportation required

Minimum Working Conditions:

• Ability to lift up to 20 lbs occasionally
• Frequent walking, standing, and computer use
• Occasional travel between locations
• Work in both office and community settings, with potential exposure to healthcare-related environments
