Job Summary:-The Social Worker works with the psychiatrist and interdisciplinary team in developing and managing treatment plans.-Serves as liaison in relation to patient care and needs, between the hospital, community, and insurers to facilitate access to clinical treatment for individuals.-Provides social services to both adult and child/adolescent patients.-Services include but are not limited to bio-psychosocial assessments, discharge planning, individual and family education and group.-Therapy Social Workers provides individual, conjoint, group and family psychotherapy; in addition to crisis intervention services, as needed.-Ensures that the quality of care is safe, effective, patient centered timely, efficient and equitable.-Social workers also serve on designated committees and as qualified, may serve as field instructors for social work.Required Skills & Experience:-Clinical knowledge and analytical abilities necessary to formulate effective social work care plans.-Ability to perform a wide variety of professional social work services.-Ability to effectively communicate with patients, families, and hospital personnel.-2-3 years social work experience-Current LMSW certification-Previous travel experience-Previous recent inpatient behavioral health experience-Experience working with multidisciplinary teams-Ability to perform intake assessment-Experience leading groups-Experience charting in EMR/EMH systemPreferred:EPIC charting experience, Discharge planning experience.Required Education:-Graduation from an accredited school of social work with a master's degree.Required Certification & Licensure:-Current Licensed Master Social Work.
