About the RoleWe're seeking a compassionate, client-focused Licensed Clinical Social Worker to join our team in a hybrid role, blending in-person and remote work. You'll provide both therapeutic and administrative support, helping clients achieve improved mental health and overall wellness in an integrated care setting.

Key Responsibilities

• Conduct comprehensive client assessments, including interviews and history reviews
• Create and implement individualized treatment plans with measurable goals
• Collaborate with the care team to provide updates and coordinate services
• Deliver crisis assessments and initiate interventions as needed
• Facilitate referrals to healthcare providers and community programs

• Maintain accurate and timely clinical documentation
• Support administrative and care coordination activities

Qualifications

• Master's degree in Social Work (MSW)
• Active LCSW license in New York
• Enrolled in NY Medicare/Medicaid (required)
• 1–2 years of clinical mental health experience
• Bilingual (English/Spanish) strongly preferred
• Comfortable with hybrid work (some in-person attendance required)
