Provides clear and concise medical record documentation.Develop and monitor enteral and/or parenteral nutrition support regimens in conjunction with pharmacy and the medical staff.Collaborates with Speech Therapy regarding diet progression and transitional feeding to meet nutritional needs.Clarify admission orders to ensure nutrition support, diet and oral supplement orders are consistent with the facility's established nutrition support formulary and diet manual.Consider religious, cultural and ethnic factors in development of nutrition care plans.Evaluate potential food/drug interactions, and/or herb/supplement interactions. Provides nutrition education/counseling as appropriate.Develop and implement educational programs for patients and staff. Works with the patient and family to identify educational needs, achievable goals and provides patient education. Serves as a resource to the medical staff regarding evidence-based nutrition practice.Attend Interdisciplinary team meetings and communicate patients' nutrition care plan, progress and recommendations. Participates in interdisciplinary team rounds i.e., wound care, vent weaning.Participate in departmental and facility Performance Improvement Program, including data collection and analysis.Serve as a resource to food service staff to assure the patient care plan is implemented. Advises food production staff regarding any special preparation techniques to be used for modified diets.Participate in developing, planning and standardizing menus. Reviews and approves general and therapeutic menus. Reviews and approves menu substitutions. Performs meal rounds and tray audits to ensure patient meal trays are consistent with the prescribed diet and meet standards of quality, quantity, temperature and appearance.Review yearly updates and approves therapeutic diet manual annually. Educates clinical and food service staff on location of the Nutrition Care Manual. Maintains hard copy of simplified diet manual for downtime.Keeps current with nutritional practices and theories. Maintains current professional registration requirements (i.e., Professional Portfolio, self-development and continuing education). Demonstrates interest in continuing education, academics or specialized certification to further enhance professional skills.Communicate patients' needs to appropriate team members (i.e., pain and suctioning). Acts as a liaison between the patient, multi-disciplinary team, medical staff and food service.Performs other duties as assigned.Additional Qualifications

  • Must be a Certified Nurse Aide
  • Must have home health or hospice patient experience
  • Patient population seen is a mixture of home bound and patients within a facility. Average of 6 patients per day and approximately 1 hour visits per patient.
