Position Summary: The Physical Therapist is responsible for providing patient care including assessing, interpreting, planning and implementing physical therapy treatments. The Physical Therapist will also maintain positive working relationships with other health professionals and ancillary staff.

Key Responsibilities:

  • Assesses patient condition, establishes goals and treatments in conjunction with the patient, and develops treatment plan.
  • Codes procedures and PT diagnoses appropriately.
  • Verifies required authorizations have been obtained prior to initial evaluation or treatment session.
  • Documents findings, treatments and other patient information in medical record.
  • Therapist will become competent in the operation and navigation of the E-EClinicalWorks EMR software. He/she will ask questions of the staff or E-EClinicalWorks staff as needed to gain competence.
  • Supervises Physical Therapist Assistant when appropriate to ensure patient care follows treatment plans.
  • Properly operates therapeutic equipment and devices including exercise equipment, modality machines, manual therapy tools, etc. Asks questions of the owner other staff as appropriate to ensure equipment is not damaged and remains in proper working order.
  • Ensures that appropriate treatment is performed in interdisciplinary fashion, consulting with physicians and others healthcare practitioners re: patient health status.
  • Adheres to CMS reporting standards, including current MIPS related standards.
  • Complies with Clinic policies and procedures according to the employee manual provided.
  • Provides patient education on care issues and use of various therapies and exercises.
  • Serves as information and education resource for patients, families, physicians, and staff.
  • Provide additional services as determined by mutual agreement with the Medical Director and Executive Director.

Qualifications:

  • Current, valid license to practice in the State of Oregon or eligibility to obtain an Oregon license.
  • Possess the tact necessary to deal effectively with patients, physicians, and employees.
  • Current BLS certificate.
  • Current, relevant, and substantial knowledge of areas related to health center licensing and operations, e.g., legal/regulatory, administrative, policy, protocols, and other clinical issues, especially within a FQHC environment, required.
  • Demonstrated ability to work effectively both independently and as part of a team in collaborative settings.
  • Commitment to maintaining the highest level of confidentiality.
  • Sensitivity to the needs of a culturally diverse patient and employee population.
  • Willingness and ability to work some evenings and weekends as needed.
  • Pleasant, professional attitude toward patients, customers, co-workers, and supervisors.
  • Strong teamwork skills.
  • Clear from any State or Federal exclusion.

Physical Requirements:

  • Duties require extensive standing and walking.
  • Requires lifting, positioning, pushing, and/or transferring patients.
  • Duties require frequent reaching, stooping, bending, kneeling, and crouching.
  • Must be able to lift and/or move up to 25 pounds occasionally.
  • Work Monday through Friday, 8:00 a.m. – 5:00 p.m., with occasional extended hours and Saturdays.
  • Regularly required to walk, talk, bend, lift, carry, write, and hear.
  • Frequently required to stand, walk, sit, and use hands to finger, handle, or feel.
  • Specific vision abilities required include close vision, distance vision, color vision, depth perception, and ability to adjust focus.
