We are currently seeking a skilled and compassionate Physical Therapist to join our growing outpatient rehab team in Elk Grove, CA. In this role, you will provide exceptional hands-on therapy, help patients regain strength and mobility, and contribute to a team culture focused on clinical excellence and patient-first care.

• Conduct comprehensive patient assessments and develop individualized treatment plans
• Deliver safe, evidence-based physical therapy services tailored to patient goals
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Supervise physical therapy assistants and aides to ensure quality of care
• Collaborate with physicians, patients, families, and care teams to optimize outcomes
• Maintain compliance with state practice acts and documentation standards
• Attend staff meetings, trainings, and contribute to ongoing clinical improvement

• Degree from a CAPTE-accredited Physical Therapy program
• Active California PT license (or eligible for licensure)
• Current CPR certification
• Excellent interpersonal and communication skills
• Strong documentation and EMR skills
• Ability to lift up to 50 lbs and assist with patient mobility
• A passion for helping others regain independence and quality of life
