Job Summary:The Physical Therapist is responsible for evaluating physical therapy needs and delivering comprehensive, medically prescribed treatment plans within a supervised rehabilitation program. The Licensed Physical Therapist will establish and administer treatment programs with specific, measurable goals that align with the patient's capacity and tolerance, while ensuring realistic and achievable outcomes.

Responsibilities:

• Conduct initial assessments and evaluations of physical therapy needs for assigned patient populations.
• Review medical charts to gather relevant medical information and clarify referrals.
• Select appropriate assessment tools and methods to accurately determine patient status and develop optimal treatment plans.
• Implement and document physical therapy treatments, monitoring patient progress and adjusting plans as necessary.
• Provide patient and family education on physical therapy methods and techniques.
• Develop and implement discharge plans in collaboration with patients, care partners, and interdisciplinary team members.
• Maintain thorough and accurate documentation of patient conditions, progress, and treatment outcomes.

Qualifications:

• Bachelor's degree in physical therapy (or higher).
• Valid New York State Physical Therapy License.
• Strong communication and interpersonal skills.
• 0–1 year of professional experience as a Physical Therapist.
