Position Summary: We are seeking two experienced and compassionate Registered Nurses (RN) specializing in Maternal and Pediatric Home Health Care. This position is field-based, serving patients in Brooklyn, NY, and involves providing in-home nursing care to individuals from newborns to 18 years old, with occasional maternal postpartum care.

Key Responsibilities:

• Deliver comprehensive nursing care to pediatric and maternal patients in a home setting.
• Conduct physical assessments, administer medications, perform wound care, and educate families on care plans.
• Manage documentation and update patient records accurately.
• Travel to patients' homes as scheduled, ensuring timely and quality care.
• Maintain a patient-centric approach in diverse and culturally sensitive communities.
• Provide care to both newborns and postpartum mothers (counted as two patients when seen together).

Required Skills & Experience:

• Minimum 2 years of recent acute care experience in one or more of the following:
  • Med-Surg
  • Maternal and Child Care
  • Critical Care
  • Pediatrics
• Current Maternal Child Home Health experience required
• Comfortable working independently in a home setting
• Strong communication and critical thinking skills

Licensure & Certifications:

• Valid New York State RN License (Primary Source Verification required)
• AHA BLS Certification (American Heart Association)
• PALS Certification (Pediatric Advanced Life Support) – required
