Tallavera is seeking a dedicated and experienced Home Care RN (Registered Nurse) to join a well-respected home care company located in Monroe County. You'll coordinate and provide skilled nursing care in accordance with physician orders while supporting patients and families throughout the care process.

Job Details:

• 13-week contract
• Monday-Friday, NO WEEKENDS!
• Rate: $52/hour
• Mileage Reimbursement

Key Responsibilities:

• Manage full patient caseloads and care plans.
• Provide skilled nursing visits, assessments, and treatments.
• Supervise LPNs and HHAs; ensure quality, safe care.
• Conduct admissions, develop care plans, and evaluate home environments.
• Teach and counsel patients and families.
• Ensure compliance with state/federal regulations and documentation standards.

Qualifications:

• Current NYS RN license required.
• Associate's in Nursing required; BSN preferred
• 1+ year home care experience.
• Knowledge of payer guidelines and infection control.
• Strong communication and critical thinking skills.
• Oasis experience strongly preferred
