Tallavera is currently seeking a Registered Nurse (RN) Home Care Case Manager in Rochester, NY. Preferred candidates for the Registered Nurse (RN) Home Care Case Manager position will be a New York State Licensed Registered Nurse, with at least 1 year experience as a clinical nurse in a hospital or other direct patient care clinical setting.

Responsibilities of the Registered Nurse (RN) Home Care Case Manager:

• Assesses and charts observations of the patient's condition at each visit
• Revises plan of treatments and discharge plan in conjunction with registered nurse case manager, patient, family, physician, care delivery team, and third-party payors including obtaining medical orders and approval of medical services.
• Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.

Requirements of the Registered Nurse (RN) Home Care Case Manager:

• NYS driver's license
• NYS Licensed Registered Nurse
• OASIS (The Outcome and Assessment Information Set) experience preferred
• Home Care experience
