Job SummaryWe are seeking a dedicated and compassionate Nurse Practitioner for our Home Health division. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while delivering patient-centered care. Must be Board Certified in Family Practice, Internal Medicine or Geriatrics.

Schedule & Hours

• Contract dates: Start Oct 15
• Day shift only
• Hours of operation: 8:00 AM – 8:00 PM
• Providers scheduled for 8-hour shifts within this window (includes drive time + charting)
• 5 days/week; preferred schedule Mon–Fri, but open to alternative 5-day blocks (e.g., Wed–Sun)
• Target 5–6 patient visits/day (may schedule up to 8 to buffer cancellations)

Travel Requirements

• Must be open to traveling throughout Michigan, covering various counties as visit volumes are completed.
• Assignments will generally focus on one county at a time (not moving between multiple counties in a single day/week).
• Primary launch area: Kent County, MI (highest member volume).
• If provider is local to Kent County, client may assign only Kent County coverage.
• Travel includes drive time, which is part of the 8-hour workday.

Key Responsibilities

• Conduct comprehensive patient assessments, establish diagnoses, and develop individualized treatment and care plans.
• Prescribe and manage medications and treatments in accordance with state scope of practice and collaborating physician agreements.
• Monitor and evaluate patient progress, adjusting care plans as needed, and escalating concerns appropriately.
• Educate patients and families on disease management, medication adherence, and home care practices.
• Collaborate with physicians, nurses, therapists, and case managers to coordinate holistic care.
• Maintain accurate, timely medical documentation in compliance with HIPAA regulations.
• Utilize EHR systems for patient management.
• Provide compassionate support for patients across diverse populations, including geriatrics, pediatrics, hospice, and palliative care.

Requirements

• Valid Nurse Practitioner (NP) license and certification in the state of Michigan.
• Board Certified in Family Practice, Internal Medicine or Geriatrics
• Experience in home health or similar clinical settings preferred.
• Strong knowledge of medical terminology, anatomy, physiology, pharmacology, and advanced clinical practices.
• Proficiency with EMR/EHR systems
• Excellent diagnostic, prescribing, and care management skills across diverse patient populations.
• Familiarity with CPT and ICD coding, as well as regulatory compliance standards, preferred.
• Strong communication, critical thinking, and leadership skills.
• Must pass background checks as required by company policy.
