Key Responsibilities

• Perform comprehensive assessments and manage chronic and acute conditions
• Develop individualized care plans in collaboration with facility teams
• Prescribe medications, order diagnostic tests, and interpret results
• Educating patients and families on health maintenance and disease prevention
• Participating in chronic care management and quality improvement initiatives
• Document all care accurately in our electronic medical record (eCW)

Qualifications

• Master's degree in nursing from an accredited NP program
• Active, unrestricted New York State NP license
• DEA license (or eligibility)
• Medicare/Medicaid provider eligibility

Preferred:

• 3+ years' experience in geriatrics or long-term care
• Familiarity with eClinicalWorks (eCW)
