Nationwide Therapy Group is seeking a LPN / LVN for a local contract job in Rochester, New York.

Position: Licensed Practical Nurse (LPN) – Long Term CareShift: 5x8-hour shifts (Evenings/Nights, 40 hrs/wk)

• Evenings: 3:00 PM – 11:00 PM
• Nights: 11:00 PM – 7:00 AM
• Weekends & holidays as neededLocation: 150 Highland Ave, Rochester, NY 14620Contract Length: 17 weeks (Guaranteed Hours)Accepting Locals: Yes

• Setting: Geriatric Long-Term Care
• Patient Ratio:
  • 1 RN : 20 Residents
  • 1 LPN : 8–10 Residents
  • 1 CNA : 4–6 Residents
• Patient Population: Geriatric residents requiring long-term care, including wound care.

• Provide direct patient care in a long-term care environment.
• Administer medications and treatments as ordered.
• Monitor and assess residents for changes in condition.
• Collaborate with and provide oversight for CNA colleagues.
• Ensure care is delivered with dignity, compassion, and respect.
• Maintain accurate documentation in compliance with facility and regulatory standards.
• Exercise strong critical thinking and independent judgment in resident care.

• Active NY State LPN License (Required).
• BLS Certification – American Heart Association only (Required).
• Minimum 2 years of experience in a long-term care setting (Required).

• Strong clinical assessment and wound care skills.
• Ability to work independently while supporting a care team.
• Excellent communication and interpersonal skills.
• Compassionate and patient-centered approach.
