Our experienced interdisciplinary team of nephrologists and renal nurses offer chronic dialysis services in a comfortable and modern setting. Located on the Queens-Nassau boarder, Queens-Long Island Renal Institute has a longstanding reputation for the finest quality of care, state-of-the-art technology and uncompromised dignity.The Social Worker provides services in the dialysis unit to patients and their families to support and maximize the social functioning and adjustment of the patient with End Stage Renal Disease.Key Tasks Include: Conducts initial patient psychosocial assessment and comprehensive plan of careProvides support, information and assistance in treatment planningDevelops and continues to monitor psychosocial goals for plan of careCounsels new patients and families on pre-dialysis education, including treatment options Acts as advocate on behalf of patient and familyCompletes and documents initial and ongoing comprehensive assessment, evaluations and quarterly notes, along with documenting and monitoring ongoing psychosocial issues and patient goals Facilitates and/or refers patients and families to support/education groupsFacilitates education on Advance Directives and treatment modalitiesCoordinates and participates in patient/family care conferences with interdisciplinary teamAnalyzes and provides information and facilitates referrals to community, private, state and federal agencies based on psychosocial needsProvides crisis intervention expertise as necessaryProvides assistance to patient and family in adjusting to dialysis and altered lifestyle, and provide resource informationArranges patient travel arrangements for both guest and permanent patients and ensure completion of arrangementsCoordinates patient transfers/ visit to other facilitiesProvides assistance to patient regarding health insurance Maintains patient confidentiality and administers patient care in a respectful mannerPosition Requirements: Licensed Social Worker in New York State; LMSW required; LCSW a plus At least two (2) years of prior work experience as a Social WorkerPrior experience as a Dialysis Social Worker or working with dialysis patients Strong interviewing skills for patient evaluation, problem solving, and recordingPossess strong analytical skills to assist patients in problem solving and resolution Capable of communicating clearly and effectively with all types of physically and mentally challenged patients and their families Professional, flexible and a team playerThis position offers a competitive rate of pay supported by an excellent benefit package including health insurance, 401(k), paid time off, free parking and more.Qualified applicants are encouraged to apply immediately!
