Qualifications:

  • Education: Completion of an accredited Licensed Practical Nursing (LPN) program.
  • Licensure/Certifications:
    • Active New York State LPN license.
    • Proof of Hepatitis B immunity (required).
  • Experience: 1+ year of experience in long-term care.
  • Soft Skills: Strong communication, adaptability to rotating shifts, and teamwork.

Key Responsibilities:

  • Administer medications and treatments as prescribed.
  • Monitor and document patients' vital signs and health status.
  • Collaborate with RNs, physicians, and therapists to implement care plans.
  • Assist residents with activities of daily living (ADLs) and mobility.
  • Ensure compliance with infection control and facility protocols.
  • Respond to emergencies and provide timely interventions.
  • Maintain accurate and timely medical records.
