Pay Range: $55 to $60/Hour.

Job description:

Provides social work services to patients and families to assist them in continuity of care planning and in enhancing their capabilities of coping with illness and daily functioning.

Collaborates with Hospital, and community-based personnel to ensure efficient service delivery and a high level of customer satisfaction.

Discharge planning experience needed.

Education:

Master s in social work

Licenses and Certifications:

LMSW required, LCSW preferred.
