GENERAL DESCRIPTION Under direct clinical supervision of the Assistant Director or Intensive Clinical Services, provides LMSW/BIS services of a habilitative nature to ameliorate or limit disabling conditions, diseases, or illnesses through the provision of professional assessments and support to persons throughout the catchment area of Mozaic. These individuals, who because of their developmental disability, require such services to remain in or move to the least restrictive residential and/or day setting. The focus of services is to help individuals and families adjust to change in their environments and programs using applied behavioral approaches. PRIMARY DUTIES AND RESPONSIBILITIES: 1. This position provides LMSW/BIS services primarily in Yates County, although this position may provide services in other Counties as needed (Cayuga, Seneca). 2. Under the Supervision of the Assistant Director of Intensive Clinical Services (ICS), the LMSW/BIS1 assures the development and implementation of Behavior Support Plans (BSP), Interaction Guidelines (IG), and Medication Monitoring Plans (MMP - as needed) including all required elements (i.e. Functional Behavioral Assessment [FBA]) in consultation with program staff and the individual's Treatment Team. (BSPs, IGs and MMPs will be referred to as Plans in this Job Description.) 3. Revise Plans periodically as required through OPWDD, on a semi-annual basis, or as necessary. Creates new Plans promptly as needed, gathering history, data, and information required for an effective Plan. 4. Collaborates with other Agency departments to oversee the process, implementation, and organization of these Plans. 5. Implements data tracking systems as needed and conducts monthly data reviews as necessary. Attends psychiatric consultations with individuals and provides pertinent information as needed. 6. Provide in-service training, education, and support to staff regarding individuals with a Plan. Ensures that staff are trained to implement these Plans. Training may include topics such as psychiatric diagnosis, support techniques, social skills training, human growth and development, etc. 7. May provide Social Work therapy to individuals/groups/families as needed and requested. Participates in the treatment review process as necessary. 8. Documents individual's progress and ensures completion of other record-keeping tasks as per Agency/program policies and procedures. Completes all clinical billing as per Article 16 Clinic policies and procedures. 9. Participates in the admission and discharge process according to Agency policies and procedures at assigned sites. Makes referrals to alternate programming as necessary. 10. May develop assessments or recommendations to provide information for Teams on an individual's adaptive behavior, cognitive, social, personality, and emotional factors. Will provide staff training as needed. 11. At the discretion of the Assistant Director of ICS, may conduct on-site observations to develop supportive interventions, and train staff on the utilization of person-centered techniques. 12. Utilizing appropriate Electronic Health Records, stores Plans (above), gathers information and electronic signatures, communicates, and compiles data tracking for review by responsible parties as needed. 13. Provides consultation and staff training on behavior management or modification techniques, social skills, human growth and development, individual's rights, and other areas as required per Agency policy and OPWDD regulations as needed. 14. Provides advocacy for individuals to ensure that their rights are not violated and needs are met. 15. May provide services at non-traditional business hours at off-site locations. 16. Participates in Quality Assurance activities as assigned. Serves on additional Agency or program committees as needed. 17. Documents individual's progress and ensures completion of other record-keeping tasks per Agency or program policies and procedures as needed. 18. Participates in training opportunities to enhance, acquire, and develop knowledge of cognitive and developmental disabilities. 19. Completes Incident Reports per agency policy. Provides follow-up for Incident Reports/Incident Review Committee. 20. May assume additional responsibilities at the discretion of the Assistant Director of Intensive Clinical Services. TRAINING REQUIREMENTS: Although the required training may vary by position, all staff members are required to receive initial training upon hire and/or within 90 days in the following: Agency Orientation, Characteristics of Disabilities, Human Growth and Development, Pro-Active Support, Corporate Compliance, Confidentiality, HIPAA Privacy and Security, Health and Safety, First Aid and CPR Annual updates are required in Pro-Active Support, Corporate Compliance, Health and Safety; Biennial updates are required for First Aid and CPR; Updates as changes occur are required for HIPAA Privacy and Security SUPERVISION: Received: Assistant Director of Intensive Clinical Services Provided: None MINIMUM QUALIFICATIONS: Masters Degree in Social Work, licensed and currently registered as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LMSW) by the New York State Department of Education; or Board Certified Behavior Analyst (BCBA). Must have training in assessment techniques and behavioral support plan development. Requires experience in working with challenging behavior, writing behavioral plans, facilitating groups, provision of Social Work services to families or case management, training in assessment techniques and behavioral program development, and five (5) years of experience with cognitively or developmentally disabled individuals preferred, or equivalent experience with non-intellectually disabled individuals or families.
