**Four (4) year grant funded position**

Job Function: As part of the SAMHSA Children's Mental Health Initiative grant in partnership with Cortland County Mental Health Department, this position is responsible for the coordination, development, implementation, and evaluation of Cortland County's System of Care (SOC) and providing participant-facing support services, resources, and referrals for families with children or adolescents struggling with serious emotional disorders (SED) and who are receiving services through the grant.

Essential Duties and Responsibilities

  • Work closely with the Cortland County Mental Health Department on the implementation of the SAMHSA Children's Mental Health Initiative grant, ensuring all CAPCO and County expectations and policies are followed along with grant standards and deliverables.
  • Implement service delivery process through programs, community outreach, and case management with families who have children/adolescents with serious emotional disorders (SED).
  • Make impactful connections with families in our community receiving services and/or seeking services and/or assistance with child/adolescent SED challenges.
  • Provide reports to the Family Development Director as required, with all necessary information to include specific program progress, needs, and problems.
  • Keep the Family Development Director and County Mental Health officials advised on unified community concerns and issues on a regular basis through scheduled meetings.

Maintain an efficient system of data collection and flow

  • Plan, develop, and implement innovative approaches within the community including programs geared toward youth and families.
  • Work cooperatively with other CAPCO programs and community agencies, including the County Mental Health Department and other local, regional, and federal government entities to create a more effective flow of service in the community.
  • Responsible for working with community groups and agencies to identify issues and appropriate solutions.
  • Possess functional knowledge of other local, state, and federal programs relating to target population, to include but not limited to families & children with serious emotional disorders, low-income individuals & families.
  • Participate in training programs which relate to and/or are required for job responsibilities and per grant standards.

Education and/or Experience

High School Diploma/GED required. Associate's degree (A. A.) preferred or equivalent from two-year College or technical school; plus, three years' experience in social work, human services or related field with youth with serious emotional disorders (SED); or equivalent combination of education and experience.

Experience as a parent or caregiver of a child or adolescent with SED who has received or is currently receiving services from the mental health system preferred. Experience in developing and coordinating community initiatives to provide maximum participation through planned outreach activities.

Other Skills and Abilities

Experience in working with individuals, neighborhood groups (youth and adults) and community organizations. Ability to grasp the problems confronting low-income residents, and families experiencing trauma and SEDs. Working knowledge of the family development process.

Certificates, Licenses, Registrations

Valid NYS driver's license with clean driving record. Must be able to pass a pre-employment background check.
