EMPLOYER INFORMATION: Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities. PURPOSE: The Lead Care Coordinator will provide leadership and oversight of the Care Coordination team and collaboration with other agencies and community services. KEY RESPONSIBILITIES: The essential functions of this position, include, but are not limited to: - Determining eligibility of referrals of Care Coordination services. - Ensuring completion of prescribed forms following standardized procedures and timelines for all Care Coordination referrals. - Maintaining waitlists for Care Coordination services. - Acting as liaison with schools, DCF, and other community agencies that provide consultation regarding local Systems of Care matters. - Assisting supervisor with support and feedback to , and consultation with, Care Coordination staff. - Assisting Care Coordinators day-to-day and in crisis situations. - Coaching and teaching the wraparound process in-house and in the community. - Providing ongoing feedback and support to Care Coordinators. - Participating in statewide supervisor meetings with DCF contract manager. - Ensuring and maintaining development of a balanced Child and Family Team. - Assisting in quality assurance review of all applicable documentation and record. - Performing other related duties as directed.

Qualifications
