Position Summary: The Office Assistant / Intake Coordinator plays a key administrative role in ensuring a smooth, efficient, and welcoming intake process for new patients. This position requires excellent organizational skills, attention to detail, strong communication abilities, and a compassionate demeanor. You will serve as the first point of contact for referrals, patients, and families while supporting the clinical and administrative team in day-to-day operations. Key Responsibilities:
- Receive and process all incoming referrals (fax, CarePort, email, phone, etc.)
- Verify patient insurance and eligibility for home health or hospice services
- Coordinate initial patient intake, including consent forms and physician orders
- Input accurate patient data into the EMR system
- Communicate with referral sources, physicians, and clinical staff to ensure timely admission
- Schedule patient evaluations and visits with nursing and therapy staff
- Maintain organized records and ensure compliance with HIPAA and CMS regulations
- Answer phones, greet visitors, and assist with general office duties
- Provide support for audits, surveys, and other regulatory needs
- Assist in ordering supplies and managing patient charts
- Collaborate with the interdisciplinary team to ensure continuity of care
Qualifications:
- High school diploma or equivalent; associate's degree preferred
- Minimum 1 year of administrative or healthcare experience (home health or hospice experience strongly preferred)
- Knowledge of Medicare, Medicaid, and commercial insurance
- Familiarity with EMR systems (Home Health Pro, Axxess, or similar is a plus)
- Excellent organizational, communication, and multitasking skills
- Compassionate and professional demeanor
- Bilingual is a plus
