Title: Job Coach and Case Worker Reports to: CORE and Vocational Services Supervisor Job Objectives: As a Job Coach and Case Manager, this role is split between two key programs at the Mental Health Association (MHA) that support individuals in building independence and achieving personal goals. You'll deliver vocational services through MHA's Vocational Program, helping participants with job readiness, employment support, and career development. In addition, you'll provide community-based mental health services - including Psychosocial Rehabilitative Services and Family Support Services - through the CORE (Community Oriented Recovery and Empowerment) program. This position plays an essential role in promoting recovery, empowerment, and inclusion for individuals across Orange County, NY. Team members are expected to foster a respectful and culturally responsive environment that aligns with MHA's Cultural Equity Policy and the principles of WELCOME Orange. Program responsibilities and supervisory structure may be adjusted over time at the discretion of the Executive Director. Responsibilities: Service Delivery:
- Provide vocational support and community-based mental health services to a caseload of individuals participating in MHA's Vocational and CORE (Community Oriented Recovery and Empowerment) programs.
- Facilitate weekly Vocational and Educational Group sessions at Hudson House, focusing on career readiness, goal setting, and skill development.
- Lead additional job training, life skills, and recovery-focused group sessions as needed.
- Help individuals build and strengthen social and interpersonal skills necessary for independent living, with an emphasis on reaching employment and educational goals.
- Support participants in developing key communication skills (e.g., active listening, conversational engagement) and expanding their natural support networks.
- Increase participants' community engagement by helping them explore local resources, identify personal interests, and access social supports.
- Deliver individualized psychoeducation and skills coaching to promote emotional wellness, stress reduction, symptom management, relapse prevention, and engagement in healthy leisure activities.
Pre-Vocational & Employment Support (ACCES-VR)
- Provide job readiness instruction on topics such as workplace expectations, punctuality, attendance, task completion, problem-solving, and workplace safety.
- Deliver soft skills training through both individual and group formats to strengthen communication, teamwork, professionalism, and adaptability.
- Coordinate and facilitate job search activities, including resume development, interview preparation, and application assistance to support job placement and retention.
- Support participants in identifying and overcoming barriers to employment, and assist in navigating transitions between education, training, and employment.
CORE Services - Health, Wellness, and Independent Living
- Help participants build comfort and confidence when interacting with healthcare providers, including scheduling and attending appointments when needed.
- Promote health and wellness routines, including sleep hygiene, medication reminders, nutrition, and self-care strategies.
- Provide hands-on support with independent living skills, such as grocery shopping, meal planning and preparation, budgeting, grooming, and maintaining a daily routine.
- Deliver individualized psychoeducation on symptom awareness, relapse prevention, and stress management techniques to enhance autonomy and daily functioning.
Community Engagement & Systems Navigation
- Establish and maintain strong working relationships with community-based providers, including Care Management agencies, county mental health clinics, SPOA, and other systems of care.
- Collaborate with CORE referral sources, ACCES-VR Counselors, and internal teams to ensure coordinated and person-centered support for all participants.
- Participate in team meetings, case conferences, and supervision to review progress, update service strategies, and contribute to program development.
Documentation, Billing, and Compliance
- Complete timely and accurate electronic documentation of all services in line with CORE and ACCES-VR program requirements using Foothold Technology (AWARDS).
- Maintain organized and up-to-date client records, both digital and physical, in compliance with agency and funder guidelines.
- Track and report service units and complete monthly billing documentation for both CORE and ACCES-VR-funded services.
Other Duties:
- In the spirit of teamwork, answer MHA's main office phone as needed.
- Participate in MHA's committees as needed.

Requirements Qualifications: Education:
- Bachelor's degree in social work, psychology, human services, or related field (required)
Experience:
- Minimum of two years' experience providing services to individuals with a serious mental illness or substance use disorder.
- Experience providing employment support, education support or vocational assistance a plus.
- Experience using motivational interviewing and trauma informed practices.
- Experience navigating an Electronic Health Record.
- Bilingual Spanish / English highly preferred.
