Job Title: IV Pharmacist Pay: $60-$65 an hr | Bi-weekly Job Type: Onsite in Des Plaines IL 60018 Shift Options: Schedule: 10am - 6:30pm with rotating weekend once to twice per month IV Pharmacist Overview: The IV Pharmacist plays a crucial role in overseeing the continuous monitoring of infusion patients. As a vital member of the Infusion Pharmacy team, you will conduct comprehensive patient assessments to evaluate therapy suitability, identify high-risk medications, allergies, and other pertinent indications or interactions. This pivotal position involves close collaboration with internal departments to ensure seamless care coordination with home health agencies, nursing facilities, and physicians. Ensure compliance with all pharmacy laws, regulations, and sterile compounding guidelines. Maintain required IV documentation and clinical monitoring. Monitors TPN patients and makes adjustment to formulas as needed. Demonstrate thorough knowledge of the notification process for prescribers and facilities regarding drug interactions, allergies, and other adverse patient safety situations. Perform accurate dose calculations based on medication, patient condition, volume, and administration rates, and program infusion pumps. Demonstrate competency in medication occurrence reporting. Understand Long-Term Care Pharmacy/Home Infusion Pharmacy Policy and Procedure Manual and internal pharmacy policies. Process IV orders when technical support is unavailable. Answer drug-related questions about IV therapy and management promptly. Communicate with facility consultant pharmacists and clinical staff regarding IV patient management. Orient and assess competency for pharmacists transitioning from oral to IV pharmacy, as well as IV technicians and infusion pump technicians, ensuring completion of the approved Infusion Training Program. Have an excellent understanding of the GCPG, formulary initiatives, and clinical programs. Review consultant customer action request reports and other reports for facilities providing IV therapy. Maintain current knowledge of infusion drugs, therapies, aseptic techniques, infusion devices, products, and supplies. Attend continuing education seminars and annual meetings to maintain clinical pharmacy skills. Perform on-call duties as required. Work varied shifts. IV Pharmacist Qualifications: IV/TPN experience required. Current Pharmacist license active and in good standing in the State of practice required. Management experience Bachelor of Science in Pharmacy or higher PHYSICAL REQUIREMENTS The physical requirements outlined below are essential for successfully performing the duties of this position: Ability to sit or stand for 7-10 hours per day. Occasional requirement to sit, climb, balance, stoop, kneel, bend, crouch, walk, or crawl intermittently. Flexibility to work extended hours as necessary. Capability to lift and/or move objects weighing up to 50 pounds. The work environment described reflects the typical conditions encountered while fulfilling the essential functions of this role. Please note, because we are a pharmacy most of our locations are open 24 hours a day, 7 days a week and therefore schedules may change as determined by the needs of the business.
