What You'll Do

• Program management:

  Own the day-to-day execution of Collaborative Care programs, ensuring clinics and patients are supported from enrollment through outcomes tracking.

• Partner coordination:

  Act as the bridge between Integral Health and local practices, health systems, and community organizations.

• Enrollment success:

  Drive patient enrollment through outreach campaigns, tracking metrics, and solving bottlenecks.

• Data & reporting:

  Monitor key performance metrics (enrollment, engagement, outcomes, ROI) and surface insights to leadership and clinic partners.

• Process optimization:

  Spot inefficiencies, propose improvements, and standardize workflows as we scale.

• Growth enablement:

  Support new market launches and help stand up new provider partnerships.

Compensation

• Remote option:

  $20–30/hour

• Hybrid in Buffalo option:

  $22–35/hour (higher for hybrid with added local coordination responsibilities)

• Part-time with pathway to

  full-time employment

• Performance bonus eligible up to $2,000-5,000/month
