For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Our Call Center provides information about and referrals to Catholic Charities' network of programs and services, as well as to agencies and organizations outside of Catholic Charities. Opportunities for Advancement STATEMENT OF THE JOB The Intake and Referral Worker, Call Center, screens all calls/requests made to the Call Center department. Upon contact with the referral source, client or potential client, the Intake and Referral Worker will provide referral information for Catholic Charities programs that appropriately meets the clients' needs. •Obtain demographic and clinical information regarding the client's relevant history and presenting concerns. •Assess and determine the most appropriate and closest Catholic Charities clinic/program site to refer clients. •Enter all required information into Catholic Charities referral database and Anasazi if required. •Determine client's benefit package. •Obtain discharge and treatment information for all inpatient and emergency services discharges. •Suggest other resources for clients in need of alternate services. •Track and provide follow-up on current and pending incomplete referrals. •Advise the referred program or clinic of incoming assessment via email. •Participates in program planning, staff meetings, case reviews, and group services. •Conveys and participates in team-oriented work. •Performs other duties throughout Brooklyn and Queens as assigned related to essential functioning of the program. •Consult with supervisor in situations where: •There is an emergency including suicidal/suicidal ideation or risk. •Client presents as too psychotic or delusional to complete application. •Suspicious of physical or sexual abuse. •Cases of Domestic Violence QUALIFICATIONS •Associates degree in Social Work, Human Services, or related field. •2 years of experience working with low-income families is preferred •Proficient in professional social work generalist practice skills. •Independent work skills. •Excellent communication (written & oral), interpersonal, and transactional skills. •Proficient in computer software applications, specifically Microsoft Office and Outlook. •Ability to travel. •Bilingual, English/Arabic, English/Cantonese/Mandarin, English/ Haitian Creole and/or English/ Russian preferred. •Frequent sitting, standing, walking, & climbing stairs. •Ability to lift 30 lbs. •Should be able to operate a computer keyboard, mouse, & office equipment. •Ability to read printed & handwritten materials. •Ability to work flexible hours (evenings/Saturdays) as assigned. •Able to travel throughout the five boroughs as needed. •Ability to read computer screen for long periods of time. •Ability to utilize public transportation, climb stairs, walk BENEFITS We offer competitive salary and excellent benefits including: •Generous time off (Vacation/ Personal Days/ Sick Days/ Paid Holidays annually) •Medical, •Dental •Vision •Retirement Savings with Agency Match •Transit * Flexible Spending Account •Life insurance •Training Series and other additional voluntary benefits.
