A well-established and mission-driven Certified Home Health Agency (CHHA) is seeking a full-time Intake Nurse to join its team in Manhattan. Position Overview: As an Intake Nurse, you will serve as a key liaison between the agency and potential clients—ensuring timely, efficient, and compliant onboarding of new patients. This is an onsite position based at a leading Manhattan healthcare facility, requiring strong interpersonal skills and a professional, upbeat demeanor.

Key Responsibilities:

  • Conduct initial nursing assessments and intakes for patients referred to the CHHA program

  • Verify eligibility, gather medical documentation, and ensure appropriate services are coordinated

  • Collaborate with physicians, discharge planners, and interdisciplinary teams to facilitate smooth transitions to home care

  • Maintain accurate and timely documentation in accordance with agency policies and DOH guidelines

  • Communicate effectively with patients, families, and caregivers to explain services and care plans

  • Ensure all intake procedures are completed with empathy, efficiency, and a focus on service excellence

Qualifications:

  • Active RN or LPN license in the state of New York

  • CHHA experience strongly preferred

  • Exceptional communication and organizational skills

  • Strong clinical judgment and attention to detail

  • Ability to work in a fast-paced environment and multitask effectively

  • Experience with EMR systems is a plus

  • Must be polished, personable, and able to build rapport with patients and providers

Compensation: Salary range: $80,000 – $105,000, based on experience and credentials Full benefits package included
