A well-established and mission-driven Certified Home Health Agency (CHHA) is seeking a full-time Intake Nurse to join its team in Manhattan. Position Overview: As an Intake Nurse, you will serve as a key liaison between the agency and potential clients—ensuring timely, efficient, and compliant onboarding of new patients. This is an onsite position based at a leading Manhattan healthcare facility, requiring strong interpersonal skills and a professional, upbeat demeanor.
Key Responsibilities:
Conduct initial nursing assessments and intakes for patients referred to the CHHA program
Verify eligibility, gather medical documentation, and ensure appropriate services are coordinated
Collaborate with physicians, discharge planners, and interdisciplinary teams to facilitate smooth transitions to home care
Maintain accurate and timely documentation in accordance with agency policies and DOH guidelines
Communicate effectively with patients, families, and caregivers to explain services and care plans
Ensure all intake procedures are completed with empathy, efficiency, and a focus on service excellence
Qualifications:
Active RN or LPN license in the state of New York
CHHA experience strongly preferred
Exceptional communication and organizational skills
Strong clinical judgment and attention to detail
Ability to work in a fast-paced environment and multitask effectively
Experience with EMR systems is a plus
Must be polished, personable, and able to build rapport with patients and providers
Compensation: Salary range: $80,000 – $105,000, based on experience and credentials Full benefits package included
