Intake CounselorLocation: Hicksville/Hempstead (split between both locations) | Schedule: Full Time, 35 hours per week, Monday-Friday| Status: Non-Exempt | Salary Range:$45,000-55,00 The Intake Counselor is a member of the multi-disciplinary team responsible for conducting initial assessments of incoming referrals, utilizing approved screening and assessment tools in the EHR, and determining treatment recommendations. The Intake Counselor will foster community relationships and ensure timely access to care for individuals seeking services from our outpatient clinic. This role requires a commitment to FCA's mission to protect and strengthen Long Island's most vulnerable families, children, seniors, individuals, and communities through their work, and inspire others to do the same. The primary location for this position will be in Hicksville with the potential of providing services in our alternate location in Hempstead. Responsibilities
- Provide same day intake access to individuals seeking services.
- Carry a small caseload of clients and collaborate with clinical team on treatment recommendations.
- Coordinate health assessments and psychiatric evaluations with appropriate medical staff.
- Collaborate with external service providers and entities to ensure coordination of care, and make referrals to outside providers as necessary.
- Explain treatment process to participants, including the program consent for treatment, program rules and regulations, confidentiality and other consent forms.
- Maintains a listing of referral agencies and maintain contact with these agencies for ongoing recruitment and referral purposes.
- Complete and submit all agency, program and regulatory reports as well as other required documentation.
- Function as an integral member of the inter-disciplinary treatment team and participate in case conferences, team meetings, and in-service workshops as scheduled
- Additional responsibilities as assigned to support the operations of the program.
Qualifications
- Bachelor's Degree in Social Work required; Master's preferred. CASAC-T holders will be considered.
- Minimum of 1 year experience providing screening and assessment to substance use or mental health population (may include internship).
- Excellent verbal and written communication skills
- Proficient computer skills with MS Office.
- Bilingual Spanish speaking preferred.
- Valid and clean driver's license
