Scion Staffing has been engaged to conduct a search for an Intake / Patient Care Coordinator on behalf of our client, a highly regard home health care agency. This is a Part-time (20+ hrs / week) contract that is onsite based in San Francisco, CA. RESPONSIBILITIES:
- Support Partner Practices: Process and verify patient data from partner medical practices using our internal systems accurately and in a timely manner.
- Identify Discrepancies: Flag any data discrepancies or system issues to the Head of Operations for further review and investigation.
- Process Improvement: Identify opportunities that could drive efficiency for our team and our partners.
- Collaborate Cross-functionally: Be the voice of the customer and partner with Engineering and Product to drive partner success.
QUALIFICATIONS:
- A bachelor's degree or 2 years of relevant experience.
- Must have experience with patient referrals.
- Must have experience with either EMR (electronic medical records), or eClinicalWorks (ECW).
- Must have experience with EPIC, and MS Office Suite
- Experience with medical documents or data is a must
