Intake Coordinator - LAKES REGIONAL MHMR CENTER : Job Details

Responsibilities as a Intake Coordinator include:

• Assessment and verification of service coordination needs
• Service planning, referral, monitoring of service provision
• Advocacy for consumers
• After-hours crisis services
• Documentation of progress.
• Requires frequent contact with consumers, family members, and service providers. Assistance with providing collateral contacts.
• Must be able to accommodate consumer and family needs which requires candidate to work a flexible schedule, including after-hours, holidays, and weekends.
• Must have working computer skills.

Requirements: High School Diploma or GED and one year experience serving persons with intellectual and developmental disabilities. Preferred Qualifications: Bachelor's degree from an accredited college or university with a major in a qualified social, behavioral, or human services field such as psychology, counseling or social work.
