Housing Works Health Home Care Coordination program works in partnership with medical and behavioral health providers and other community-based partners with a goal of aligning care coordination services that promote access to care and enhanced health outcomes for Medicaid recipients with unstable chronic conditions. The Health Home model of care uses comprehensive care management to address medical and behavioral health needs as well as social determinants of health like housing and benefits navigation.
Intake Coordinators are an integral part of the team who are responsible for promoting the Health Home program, screening clients for program eligibility and appropriateness, and completing program intakes, while adhering to agency and program policies and procedures around intake and enrollment. The IC is clinic and community based, responsible for working with the clinic teams and the Community Partnerships teams to complete timely eligibility assessments and intakes for referred clients. The IC is also responsible for promoting the Health Home program to clinic patients, agency programs and clients, and community partners and community members via event-based engagement. Performance is measured by meeting monthly target metrics and completing required documentation.
