PRIMARY FUNCTION:

The intake coordinator is responsible for engaging and supporting youth and families from pre-admission through the admission process for the Residential Treatment Facility (RTF). The intake coordinator works hand in hand with OMH, OCFS, treatment teams, and social service providers to bring the family and child into the center of the treatment and to successfully enroll the child into treatment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Access or evaluate the high-risk factors per prospective clients.
  • Participate in clinical and treatment meetings including, but not limited to, pre-admissions.
  • Work collaboratively with multiple teams to optimize outcomes for clients and expedite enrollment.
  • Effectively and efficiently navigate and communicate with systems including, but not limited to, OMH, OCFS, DSS/ACS, DOE, etc.
  • Coordinate Meet and Greets with parent/guardian/caregiver, child, providers, and RTF staff.
  • Educate caregiver on the program and its offerings and provide timely follow-up as needed.
  • Ensure all necessary school supports and needs are met, maintaining regular contact with the RTF in-house D75 school system.
  • Ensure all necessary parties on the Senior admin team at the RTF are informed and kept up to date on client enrollment and barriers to admission.
  • Maintain confidential client information.
  • Accurately document meetings, follow-ups, and written correspondence in a timely manner.
  • Provide for and maintain tracking of all client movement throughout the admission process.
  • Facilitate group work with newly enrolled clients.
  • Ensure compliance with all OMH regulations and guidelines.
  • Other tasks as required.
EDUCATION and/or EXPERIENCE:
  • Master's Degree in social services field with at least 2 years of experience providing direct services to or linking children with SED to services.
  • Ability to write and communicate effectively.
