Intake Coordinator - Home Care - Bilingual

Preferred Home Care of New York, a fully licensed Home Care Agency serving the NY Metropolitan area including Nassau, Suffolk and Westchester counties, is seeking an experienced, Intake Coordinator for our Brooklyn Corporate Office who understands the regulations governing the home care field, related to Medicare, Medicaid and other insurances, and is committed to excellence in patient care. The Intake Coordinator will enjoy working independently and apart of a dynamic and supportive intake team.

POSITION SUMMARY

Intake Coordinators are responsible for processing referrals received from marketing sources such as hospitals, nursing homes, and community physicians. Coordinators contact each patient to educate them on the services we can provide and assist them with their homecare needs for skilled and non-skilled services. In this important position Intake Coordinators accept the great responsibility of following a patient from the date of discharge through enrollment for long term care, keeping the patient and family members informed at all times to ensure all patients have caring and professional experience with Preferred Home Care.

REQUIREMENTS INCLUDE:

Two year of experience in home care intake

High school graduate; some college credit preferred

Understanding the requirements for CHHA's and MLTC's

Understanding of communicating effectively with employee, patients and their families, medical and community affiliates in order to develop positive relationships.

Organized and punctual

Proficient in Microsoft Office Suite, Constituent Database (HHA Exchange)

Investigative ability, highly organized, self-motivated, takes initiative, excellent written and verbal communication and analytical critical thinking skills; able to perform without much supervision.

JOB RESPONSIBILITIES INCLUDE:

Receive referrals from referral sources

Verify patient's insurance to determine eligibility

Educate patients on services we provide

Send referral to MLTC for potential enrollment or Medicaid assistance

Send referral to appropriate CHHA for skilled services

Determine what services were approved by insurance

Ensure PHC receives authorization for MLTC cases and provide case specifics to PHC HHA Coordination Department
