Parker Jewish Institute for Health Care and Rehabilitation, located in New Hyde Park, NY, is seeking a full time Admissions Intake Coordinator to join our team in Admissions and serve as a subject matter expert, acting as the single point of decision making for patient admissions.
This position is an exciting opportunity to perform pre-admission clinical assessments of referred patients and to collaborate with our outstanding interdisciplinary team, providing outstanding clinical judgement to admit patients to our 5-star CMS rated facility, allowing Parker to focus on its outstanding patient centered care and allow us to continue to raise the bar in clinical excellence.
The Intake Coordinator-Admissions will play a key role by completing pre-admission assessments (PRI) in conjunction with the Admissions Assistant Vice President, to identify medical, social and financial issues with potential new admissions, and ensure that patient's needs will be met upon admission by collaborating with the Parker team. You will act as an ongoing patient liaison, serving as the initial contact for patients coming into Parker, while maintaining ongoing metrics and quality improvement initiatives.
