About the Role

As the Intake & Referral Coordinator, you'll be the first point of contact for new referrals, ensuring a smooth and welcoming start to care for our patients. You'll work closely with referral sources, patients, and our clinical team to coordinate timely admissions and support our commitment to compassionate, efficient care.

Key Responsibilities

• Manage incoming referrals and verify patient eligibility for services.
• Collaborate with referral sources, including physicians, hospitals, and community partners.
• Coordinate patient admissions with our clinical team to ensure timely care.
• Maintain accurate and organized records in compliance with healthcare regulations.
• Provide exceptional customer service to patients, families, and referral partners.

• Experience in healthcare intake, referral coordination, or a related field (home health experience preferred)
• Strong organizational and communication skills.
